INFORMATION FOR ALL FIRE CLASS STUDENTS

Items for the Student to complete:

1. (WHITE) ADD/DROP form. PRINT name, Last, First, Middle & SSAN

· Sign and date the form

2. If the student HAS NEVER attended Bakersfield College, he/she must complete a BLUE “Student Admissions Form” using a #2 pencil. Forms completed in ink will not be submitted and you will not be enrolled in the class.

3. If the student has, as some point in time, attended Bakersfield College since 1987, he/she must complete a BEIGE “Student Information Update Form”

4. If he/she is a current Bakersfield College student and has already done an “UPDATE” through the Bakersfield College web site for THIS semester (Spring 2004), you need only fill out the white add/drop form.

5. PLEASE USE A #2 PENCIL!!!!!

· The major code is 213300

· Please do not confuse your birth date with today’s date
